Hospital and Health Service Liaison

Fremantle Medicare Local works closely with Primary Care practitioners, other Medicare Locals, the South Metropolitan Health Service, the Department of Health WA and the public tertiary health service.

This facilitates representation and the involvement of Primary Care in the development of hospital and health service policy and planning of services.

The overriding goal is to optimise the continuity of care of patients as they move between primary and tertiary sectors. In line with the values and strategic objectives of Fremantle Medicare Local, this involves promoting and improving quality, collaboration, integration and innovation. More specifically, this includes a focus on

  • Area-wide mapping, identification of gaps and planning of services
  • Referral pathways, shared care and alternatives to admission
  • Access, referral and admission
  • Outpatient reconfiguration
  • Discharge planning
  • Quality communication including eHealth
  • Research, education and innovation

Fremantle Hospital and Health Service GP Liaison

Fiona Stanley Hospital GP Liaison

Scheduled to open in 2014, the Fiona Stanley Hospital will offer comprehensive public tertiary health care services to communities in the south of Perth and across Western Australia. It will provide 643 beds, plus an additional 140 beds in the State rehabilitation service. Fremantle Medicare Local is already contributing to operational design processes, particularly in the areas of discharge planning and outpatient process maps. The realignment of public tertiary hospital services across the two public tertiary sites in our catchment will be a major focus.
